Sounds like a weightless vertex, this could mean one of two scenarios:
1.) Your vertex isn't assigned to any bones and therefore has no weight.
2.) Your vertex is assigned to a bone or bones but the total weight adds up to 0.Solution:
Try selecting all your vertices and checking the weight table. Give the problem vertices some weight on at least one of the bones in your skeleton. Pick a bone closest to the location of the problem vertex. To make sure bones are always assigned to at least one bone, make sure you check Weight all Vertices
in the Advanced Parameters of the Skin Modifier. This will force vertices to be assigned to at least one bone.
